A young psychotherapist needed her practice forms branded to match the professional, approachable energy she brings to her work.
Transform standard clinical intake forms into branded materials that feel both professional and welcoming, reinforcing her practice values from the first client touchpoint.
I developed a calming visual identity using a teal color palette, clean typography, and her portrait for personal connection. Then designed templates for intake forms and client documents that were beautiful, functional, and easy to update.
